If you are interested in obtaining information about our insurance products, please call 1-888-815-3688. Our representatives can assist with questions regarding our products and how to buy them.

Q. I've lost my policy. How do I get a duplicate?
A. Call our customer service at 1-888-815-3688 and we will take the necessary information in order to get you a duplicate. If in the meantime you have any questions regarding your coverage, just ask.


Claims Processing

Q. How long from the date of the service do I have to file a claim?
A. Most of our policies provide that the insured must give written notice of a claim within 30 days after a covered loss.

Q. How do I get a claim form?
A. You can download a claim form and instructions on how to file a claim. Or you can call, write or fax us requesting the appropriate form.

Q. Where do I send the claim form to be processed?

Q. Can I fax my claim to you?
A. You can fax your completed claim form to 905-305-8600. However, please note that we will also need the signed original to be mailed to the address above.

Q. What information do I put on the beneficiary change form?
A. Download and print the Change of Beneficiary form. Complete the form by indicating the policy number and who the new beneficiaries should be. Mail the completed form to:

Q. How can I change the way I pay my premium?
A. If you are presently paying your premium through payroll deduction and would like to pay through pre-authorized chequing, you can request a change by contacting us at 1-888-815-3688 or download and print the Change Your Bank Information form. Complete the form and mail it to:
